Proofs

You need to provide one type of proof from each of the following categories -

Proof of Address

The photo or document must clearly show the applicant's name and address (printed not handwritten) and the type of document it is -

  • if using a utility bill as evidence, then the uploaded copy must show its issue date which must be within the last 12 months
  • if using a bank statement as evidence, then the uploaded copy must show its issue date which must be within the last 12 months
  • if using a council tax bill, then it must clearly indicate it is for the current year or dated within the last 12 months

Proof of Age

The photo or document must clearly show the applicant's name and the type of document it is -

  • if using a birth certificate as evidence, then your name must not have changed. If changed by marriage, then a photo/scan of your marriage certificate will also be required. If changed by deed poll, then a photo/scan of your deed poll document will be required
  • if using a driving licence as evidence it must be a valid UK photocard licence
  • if using your passport, it must be a copy of the personal details page

Proof of Disability

There are seven categories of disability in the context of entitlement for a bus pass and documentary proof is required as follows -

Blind or Partially Sighted

  • A yellow card being proof of registration with Social Services as severely sight impaired
  • An orange card being proof of registration with Social Services as partially sighted

Profoundly or Severely Deaf

  • A white card being proof of registration with Social Services as profoundly or severely deaf
  • Personal Independence Payment with a score of at least 8 points in the communicating verbally activity (please provide first page of award letter plus the further pages confirming points awarded)

Without Speech

  • A grey card being proof of registration with Social Services as being without speech
  • Personal Independence Payment with a score of at least 8 points in the communicating verbally activity (please provide first page of award letter plus the further pages confirming points awarded)

Mobility (walking) substantially impaired

  • Higher Rate Mobility Component of Disability Living Allowance (dated within the last 12 months) with a period of entitlement of at least 12 months
  • Personal Independence Payment at 8 points or more in the moving around activity (please provide first page of award letter, showing personal details and period of award, plus the further pages confirming points awarded)
  • War Pensioners Mobility Supplement
  • Disabled Person's Parking Badge (Blue Badge), which must be valid for at least six months and both sides of the badge must be provided
  • Letter from a medical specialist (not a GP) confirming that your walking ability is long term and substantially impaired.

Loss of or loss of use of both arms

  • A blue card being proof of registration with Social Services as having long-term loss or use of both arms
  • Letter from the NHS (limbless unit) confirming your condition and that you are unable to use your arms to carry out everyday tasks

Learning Disability

  • A pink card being proof of registration with Social Services as having a learning disability which commenced before adulthood with a lasting effect on development and is such that you have difficulty in learning new skills etc
  • Personal Independence Payment with a score of at least 8 points in the communicating verbally activity (please provide first page of award letter plus the further pages confirming points awarded)

Unable to Drive on medical grounds

  • Letter from the DVLA indicating that your driving licence has been refused or revoked for a minimum of 12 months
  • A current letter from your medical practitioner stating clearly and unambiguously that your entitlement to drive has been withdrawn, refused for medical reasons, or an application for a driving licence would be refused if one was applied for. The letter should also state if the condition is permanent or temporary and, if temporary, the expected duration

This category does not include those who are excluded from holding a licence due to persistent misuse of drugs or alcohol.
